Information on what you plan on donating will be used to generate a printed list that attendees at the event can refer to.
It will also be used to generate tags for each donated item.
If you wish to donate, please email your info to me and mail your entry form in as soon as you can.
Items are to be delivered to the clinic (1st floor) at 4pm on Oct 21st. This will allow us a couple hours for set up before the event starts at 6pm.
